2 Weeks Well visit (there may also be a 5-day-old well visit for first-time and nursing moms), maternal survey
2 Months Pediarix #1 (diphtheria, tetanus, pertussis, polio, hepatitis B), Prevnar #1 (pneumococcus), Hib #1 (hemophilus influenza type B), Rotavirus #1, also maternal survey
4 Months Same as two-month visit - Pediarix #2, Prevnar #2, Hib #2, Rotavirus #2
6 Months Pediarix #3, Prevnar #3, Rotavirus #3
9 Months Vaccine catch up
12 Months Hepatitis A #1, Prevnar #4, Hemoglobin, Lead
15 Months MMR #1 (measles, mumps, rubella), Varicella #1 (chickenpox), Hib #3
18 Months DPT #4 (diphtheria, tetanus, pertussis), Hepatitis A #2 (if 6 months from #1)
24 Months Hepatitis A #2 (if not yet given), Hemoglobin, Lead
30 Months Developmental review
3 Years Vaccine catch up if necessary, vision screen, hearing screen
4 Years MMR #2, Varicella #2, Kinrix (DPT #5, IPV #4), vision screen, hearing screen
5 Years Vaccines listed at 4-year visit if not completed, vision screen, hearing screen
Ages 6-21 Annual physicals
• All ages - 2nd Varicella (chickenpox) if not yet given
• All ages - Influenza vaccine - annually from 6 months and up, injection or nasal mist
• Age 10 - Boostrix (Tdap) - tetanus, diphtheria, pertussis booster
• Age 11-12 - Meningococcal vaccine #1
• Age 11 and up - Gardasil (HPV) - all boys and girls ages 11 and up, series of 3 injections
• Age 16 -18 - Meningococcal vaccine #2

  • All well visits include height and weight measurements
  • BMI assessed annually from age 3 and up
  • Head circumference is measured at all well visits through the 24-month visit
  • Urine for Chlamydia testing is recommended for girls age 15 and up if indicated
  • Hemoglobin is measured in girls one year after the onset of menstrual periods
  • Development and autism screening will be done at key ages in infancy and early childhood
