Appointments Please call our office to make any appointment. Our staff will do its best to schedule an appointment in a timely fashion. We request that you call the office for any well or sick appointment. If your child is urgently ill or injured and there is no time to call the office, you may have to consider proceeding directly to the Emergency Room or calling 911. We schedule well visits or non-urgent sick visits up to three months in advance. Please call well in advance of your desired appointment date. Children beyond the age of two years old should be seen annually for health maintenance exams. Sick Appointments are usually made on the day of the phone call. We have significant capacity to see sick children usually within several hours of the phone call. Please call first before coming in so that we may give you an exact time and so that we may get your child's record ready for the visit. We now also have a "tickler file." You may now fill out a postcard specifying your appointment requests regarding preferred doctor and time. When the schedule for the month comes out, your request will be honored and the postcard will be sent to you in the mail. Getting Prompt Medical ServiceDuring our office hours, please call the office for medical advice. Our regular phone hours are: Monday-Friday 7:30am - 5pm Saturday 8:00am - 2pm Any calls outside of these hours will be referred to our answering service. Please call our main phone number (632-8050) for instructions. Your call will be returned within an hour by an Amherst Pediatrics M.D. or by a trained triage nurse. If your child has an extremely urgent illness or injury, it may be necessary to call 911 or to proceed directly to the Emergency Room. Transferring Medical RecordsTransferring medical records works in two directions. Either records are being transferred to Amherst Pediatrics from another provider or records are being transferred from our office to another medical provider. Usually, a signed authorization from a custodial parent is required before records may be released in either direction. Our business office has authorization forms for medical record transfer. There may be a fee for some forms of record transfer. If you have any questions, please call the business office at 632-2729. Insurance
Participation in these plans may change without notice. If you do not see your insurance plan listed, please call our business office for updated information at 632-2297. School FormsSchool forms are generally required when a student enters a new school and annually for sports participation. School forms are also required for kindergarten, 1st grade, 3rd grade, 7th grade, 10th grade and college entrance. Usually, we can fill out the form if a well exam has been performed in our office within the past 12 months. Please drop off the form with a self-addressed, stamped envelope and we will return it as soon as possible. If you need a school form please have your child pick one up from the school nurse or download our generic school form. If your child has not had a well physical exam in our office in the past 12 months, please set one up as soon as possible. College physicals are the exception to the above advice. For a college physical, we do require specific school forms and do require a special college physical visit to be scheduled with the office. ReferralsIf you feel that your child needs a referral to a specialist or to an ancillary health care provider, it is advisable that you talk to your primary doctor within Amherst Pediatrics. At times, Amherst Pediatrics can provide the required service. Your Amherst Pediatrics doctor is in the best position to refer your child to the appropriate provider. Our hope is that we can coordinate the care that your child receives in the medical community. Please utilize the on-line referral form. Some insurance companies require that your primary care doctor approves any request for referral. Please refer to your insurance contract. Camp Forms If you call the office after office hours, you will be instructed to call the office during office hours. However, if your question is of an urgent nature, your call will be returned by an Amherst Pediatric doctor or by a trained triage nurse.
